Job Details
- Job Type: Full-Time / Field-Based
- Location: Area of coverage is Livingston, Onalaska, Point Blank, Cold Springs, Groveton, and Trinity
- Setting: Home Health Care
What You'll Do
- Provide coordinated physical therapy to home-bound patients across various age groups under the supervision of a Licensed Physical Therapist.
- Actively participate in patient care coordination to help patients meet their recovery goals.
- Maintain clear documentation and adapt care to individualized patient needs.
What We're Looking For
- Education: Associate degree from an accredited Physical Therapist Assistant program.
- Licensure: Active State PTA License (or multi-state license via the PTLC Compact).
- Certifications: Current CPR certification.
- Experience: 1 years of experience as a PTA preferred (Home Health experience a plus).
- Skills & Requirements: Strong interpersonal skills, ability to work independently, valid driver's license, and ability to handle physical demands (lifting, standing, local travel).
