Main Function: Supplements the nursing care needs of the client as provided by the Registered Nurse and include:
Providing services in accordance with agency policies;
Preparing clinical and progress notes;
Assisting the Registered Nurse or physician in performing specialized procedures and duties;
Assisting the Registered Nurse in carrying out the plan of care; and,
Assisting the client in learning appropriate self-care techniques.
Duties and Responsibilities:
Supplements the nursing care needs of the client as provided by the Registered Nurse and include: preparing clinical and progress notes written the day service is rendered and incorporated into the client record within 14 working days.
Shall utilize a systematic approach to provide individualized, goal-directed nursing care by: i) collecting data and performing focused nursing assessments;
ii) participating in the planning of nursing care needs for clients; iii) participating in the development and modification of the comprehensive nursing care plan for assigned clients
implementing appropriate aspects of care within LVN’s scope of practice including compliance with the laws as applicable to the LVN’s practice setting; assisting in the evaluation of the client’s responses and outcomes to therapeutic interventions and the identification of client needs; and
utilize a problem-solving approach as the basis for decision making in practice.
Notes change in client’s physical or emotional status.
Teaches and supervises the client and family members regarding the nursing care needs.
Collaborates with other team members in the care of the clients, under the direction of the referring Physician and supervision of the Registered Nurse.
Establishes priorities and goals for the client and implements plans to achieve them under the supervision of the Registered Nurse.
Administers medication as defined in the medication policy, and treatments, as prescribed by a Physician.
Submits clinical notes on a daily basis with accurate, completed itinerary.
Consults with Physician when necessary and documents, in writing, appropriate changes of orders that are necessary and notifies the Supervisory RN.
Acts as a liaison between the client, the health team, the Physician and the Agency’s administrative staff.
Participates in professional meetings when directed. Attends scheduled monthly in-services and weekly case conferences.
Notifies the Regional Supervisory RN and Parent/Branch Supervisory RN of any potential discharge so the Parent/Branch Supervisory RN can assess the client’s medical condition for appropriate discharge if applicable.
Implements discharge plans for Agency clients under the supervision of the Supervisory RN and Regional Supervisory RN.
Keeps fully cognizant of CMS reimbursement criteria, current charting needs and dates of re-certification of all clients under his/her care and Registered Nurse supervision.
Rotates working on the weekend with the other staff nurses and complies with policy and procedure for the weekend shift.
Is responsible for completing Clinical review adjustments and turning in by the determined guideline.
