Qualifications and Skills:
Registered Nurse, currently licensed in the State of Texas.
Graduated from an approved school of professional nursing.
A current CPR card preferred but not required.
One year of experience in an acute care setting that requires heart, lung, and complete physical assessment is preferred, but not required. Demonstrated ability or education in the same areas is preferred.
Responsibilities and Duties
Know and conform to the Texas Nursing Practice Act and the board’s rules and regulations as well as all federal, state, or local laws, rules or regulations affecting the nurse's current area of nursing practice.
Provides comprehensive nursing care through the utilization of nursing assessment, diagnoses, and care plans. Uses a systematic approach to provide individualized, goal directed nursing care.
Performs nursing assessments regarding the health status of the client and notes changes in the clients physical or emotional status informing the physician and other staff of changes in the client’s needs.
Administration of medications and treatments.
Performs initial and ongoing comprehensive assessments of the client’s needs including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points evaluating outcomes of care including planning for the discharge from service.
Initiates appropriate preventative and rehabilitative nursing procedures as needed.
Make assignments to others that take into consideration client safety and that are commensurate with educational preparation, experience, knowledge, and physical and emotional ability of the person to whom the assignments are made.
Supervise care of the client provided by others when the RN is administratively or professionally responsible including LVNs, paraprofessionals providing services and assigning home health aides to a specific client.
Assigns home health aides to a specific client within his/her service area or as directed by the Supervisory RN. Supervises the home health aide every fourteen days or less and documents these visits on a clinical note.
Institutes appropriate nursing interventions that might be required to stabilize a client’s condition and/or prevent complications; informs the physician and other staff of changes in the client's needs.
Clarifies any order or treatment regimen that the nurse has reason to believe is inaccurate, non-efficacious or contraindicated by consulting with the appropriate licensed practitioner and notifying the ordering practitioner when the nurse makes the decision not to administer the medication or treatment.
Makes nursing diagnoses that serve as the basis for the strategy of care establishing priorities and goals for the client and implements plans to achieve them.
Coordinate services incorporating other team members into the care of clients referring other services as needed under the direction of the referring physician.
Consult with, utilize, and make referrals to appropriate community agencies and health care resources to provide continuity of care.
Plans, develops, evaluates outcomes of care and implements discharge plans for Agency clients completing discharge change order after consulting with the Supervisory RN and Regional Supervisory RN.
Prepares written care plans/assignments for Home Health Aides and updates supervisory visit.
Rotates on-call with the other staff RN’s and complies with the policy and procedures for on-call.
Other duties as assigned by the Parent/Branch Supervisory RN, and/or Administrator.
Assists with the orientation and on-going training of nurses and home health aides.
Complies with departmental and agency policies and procedures.
