Job Summary
Community Health Worker provides preventive health improvement services by working closely with interdisciplinary care team, internal/external agencies to address social determinants of health to improve the overall health and well-being of the child and family.

Role and Responsibilities:
Connecting individuals to healthcare and social services
assistance with navigating healthcare systems
social support
Reduce healthcare disparities and enhance patient engagement by serving as a bridge between communities and healthcare systems, CHWs help ensure culturally appropriate care, empower individuals, and strengthen public health initiatives.
