Position Summary
The Patient Record Abstractor IV will review, abstract, and code medical information from cancer patient records in accordance with California Cancer Registry (CCR), SEER, California Cancer Surveillance Program (CSP) of Los Angeles County, Cancer Registry, and Commission on Cancer guidelines. The role requires a high level of accuracy, with periodic review and sampling of abstracts and an expected error rate of less than 3%.
Required Qualifications
- ODS certification required.
- 5 years of cancer abstracting experience.
- Experience working in a Cancer Registry environment.
- Minimum 2 years of healthcare experience.
- Advanced Microsoft Office skills, including Word, Excel, and Outlook.
- Experience with healthcare or financial systems.
- Knowledge of CCR, SEER, and Commission on Cancer reporting requirements.
- Knowledge of ICD-O, ICD-9, or ICD-10 coding.
- Knowledge of medical terminology, anatomy, physiology, and disease processes.
- Experience interpreting medical records and abstracting clinical and demographic information.
- CNExT database knowledge.
- Strong understanding of medical information confidentiality and release requirements.
Key Responsibilities
- Abstract and code cancer-related clinical and demographic information from medical records.
- Accurately enter abstracted information into the Cancer Data Center database.
- Review records and conduct follow-up activities for cancer registry patients.
- Maintain accurate and organized cancer registry files and documentation.
- Perform periodic quality reviews and maintain an error rate below 3%.
- Interpret medical records and identify relevant information for cancer abstraction.
- Contact patients and respond to questions when required.
- Coordinate with hospital departments to obtain necessary source information.
- Identify and report system or procedural discrepancies to the appropriate supervisor.
- Manage changing assignments, priorities, and deadlines independently.
- Maintain confidentiality of patient information in accordance with applicable regulations.
- Collaborate effectively with colleagues, medical staff, administration, and other departments.
