Case Manager – Job Description


Location: A New Well, Inc. – St. Charles, Missouri
Position Type: Full-Time
Reports To: Program Director & CEO


Position Summary

The Case Manager is responsible for coordinating services, supporting individuals and families, completing required documentation, and ensuring compliance with state regulations and A New Well, Inc. policies. The Case Manager serves as the primary liaison between individuals, guardians, interdisciplinary team members, medical providers, and state agencies (DMH, DHSS, Medicaid). This role ensures all individuals receive high-quality, person-centered services consistent with their ISP (Individual Support Plan) and treatment needs.


Essential Duties & Responsibilities

1. Individual Service Coordination

  • Serve as the primary point of contact for assigned individuals.
  • Coordinate all services, programs, and supports required under the ISP.
  • Conduct person-centered assessments, interviews, and home/program visits.
  • Attend ISP meetings, case conferences, and interdisciplinary planning sessions.
  • Ensure timely implementation of supports, goals, interventions, and action steps.
  • Advocate for individual needs, rights, and preferences.


2. Documentation & Compliance

  • Complete case notes, service documentation, and progress summaries in accordance with:
    • Missouri DMH requirements
    • DHSS and Medicaid Waiver rules
    • CARF standards
    • A New Well, Inc. policies
  • Maintain up-to-date files including medical records, consents, assessments, and ISP documents.
  • Track and document all contacts with individuals, guardians, and professional stakeholders.
  • Prepare documentation for audits, quality reviews, and licensing inspections.
  • Ensure confidentiality and HIPAA compliance at all times.


3. Monitoring & Quality Assurance

  • Conduct monthly, quarterly, and annual service monitoring as required.
  • Verify staff compliance with ISP goals, behavior plans, and support needs.
  • Complete home visits, day program visits, or community support observations.
  • Identify service gaps and make recommendations to improve quality.
  • Report concerns related to safety, abuse/neglect, or exploitation immediately.


4. Communication & Coordination

  • Collaborate with nurses, residential staff, direct support staff, physicians, behavioral providers, and guardians.
  • Communicate updates regarding health, behavioral changes, incidents, or service needs.
  • Maintain positive professional relationships with case managers and other agencies.
  • Coordinate medical appointments, follow-ups, and necessary referrals as needed.


5. Crisis & Incident Response

  • Support individuals and staff during emergencies or unexpected events.
  • Assist with completing incident reports and follow-up documentation.
  • Participate in investigations as directed by leadership.
  • Ensure corrective action plans are completed and documented.


6. Program Support

  • Participate in staff trainings, meetings, and professional development.
  • Provide guidance to staff on ISP goals, behavioral strategies, and person-centered approaches.
  • Assist with intake coordination by gathering documentation and completing screenings.
  • Support individuals transitioning into or out of services.


Qualifications

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field preferred.
  • Associate’s degree with experience may be considered.
  • Prior case management experience in disability services, healthcare, or community programs preferred.
  • Knowledge of Missouri DMH, DHSS, and Medicaid Waiver programs strongly preferred.
  • Strong writing, documentation, and organizational skills.
  • Ability to work independently and manage multiple cases at once.
  • Valid driver’s license and ability to travel locally.
  • Ability to maintain confidentiality and practice trauma-informed, person-centered care.

Physical Requirements

  • Ability to sit, stand, walk, drive, and conduct home/program visits.
  • Ability to lift 10–20 lbs (documents, supplies).
  • Ability to respond quickly in emergencies.


Work Schedule

Monday–Friday, with some flexibility for meetings, appointments, and emergencies.
Occasional evening or weekend work may be required.
