Job Title: LTSS Assessor - Registered Nurse (RN) – Field-Based Care Management
Position Type: Permanent / Direct Hire

Territory Covered: Steuben, Chemung, Schuyler, and Yates Counties
Schedule: Monday–Friday (Field-based)
Compensation: $35.49 – $63.79 per hour based on licensure, education, and experience
Bonus: 8% Annual Incentive Opportunity
Position Overview
This is a field-based Registered Nurse role focused on conducting UAS Assessments and Reassessments for complex and high-acuity populations. The RN will assess, coordinate, and manage medical and supportive services across the continuum of care, ensuring high-quality, cost-effective, and person-centered outcomes.
Candidates must reside in Steuben County and be willing to travel within the assigned counties to complete 2–4 UAS assessments daily.
Key Responsibilities
- Conduct in-home and site-based UAS assessments and reassessments
- Evaluate service needs of high-risk, high-acuity members
- Develop, implement, and continuously reassess individualized care and service plans
- Coordinate care between members, caregivers, providers, and community resources
- Monitor member status, changes in condition, and progress toward care goals
- Identify trends in member data to support quality improvement initiatives
- Educate members and caregivers on disease processes, care options, and available benefits
- Act as a member advocate and liaison across interdisciplinary teams
- Document all care management activities in compliance with state and federal regulations
- Support quality improvement initiatives and cost-effective service delivery
- Precept and mentor new clinical hires as needed
- Collaborate with leadership and cross-functional teams
- Perform other duties as assigned
Required Qualifications
- Active New York State RN License
- Minimum 4–6 years of relevant RN experience
- Experience in the healthcare industry, specifically in care management or a related field, is required. The ideal candidate should have experience working with complex/high acuity populations and be familiar with state and federal regulations.
- Residency in Steuben County, NY
- Valid driver’s license
- Personal transportation preferred (public transportation will be considered)
- Strong assessment, care coordination, and clinical judgment skills
- Excellent communication and patient education abilities
- Graduate from an Accredited School of Nursing or Bachelor’s Degree in Nursing (preferred)
Preferred / Nice-to-Have Qualifications
- Care management experience within a Managed Care Organization (MCO)
- Experience working with complex or high-acuity populations
- Knowledge of community-based resources and service coordination
- Leadership experience, including mentoring or precepting clinical staff
- Familiarity with state and federal healthcare regulations
