Alexis Wordlaw-Conley awordlaw-conley@alinestaffing.com RN Nurse Case Manager–position DetailsLocation : 100% On-Site –Virginia (Prefer these locations: Alexandria, Manassas, Fairfax, Arlington, Springfield, or surrounding areas)Schedule: Monday - Friday 8:00 am - 5:00 pm EST.Pay Rate: 44.14RN Nurse Case Manager–you will:· Utilize a collaborative process of assessment, planning, facilitation, and advocacy for health needs.· Conduct comprehensive assessments of referred members’ needs and determine case resolution approaches.· Apply clinical judgment to develop strategies addressing complex health indicators.· Perform crisis intervention with members experiencing behavioral health or medical crises.· Monitor and document care plans in compliance with regulatory and accreditation guidelines.· Engage with healthcare professionals to influence care decisions and enhance member engagement.RN Nurse Case Manager–RequirementsActive and unrestricted RN license in Virginia or compact license required.Case Management Certification (CCM) preferred.3 years clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required.Experience preferred managing chronic conditions, such as diabetes, hypertension, and asthma.Healthcare and/or managed care industry experience required.Case Management experience preferred-- Position requires proficiency with computer skills which includes navigating multiple systems and keyboardingEffective communication skills, both verbal and written.Ability to multitask, prioritize and effectively adapt to a fast paced changing environmentSedentary work involving periods of sitting, talking, listening.
