Field Community Health Worker Compensation
The pay for this position is $25 per hour
Benefits are available to full-time employees after 90 days of employment
A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Field Community Health Worker Highlights
This position is a contract-to-hire assignment based on attendance, performance, and business needs
Required availability for this position is Monday-Thursday, 9 : 00 AM - 6 : 00 PM, and Friday, 8 : 30 AM - 5 : 30 PM
Training will be held in Atlanta, GA , for approximately one week
This role requires 80-95% travel within the state of Georgia
Field Community Health Worker Responsibilities
Conduct home visits with high-risk members to assess barriers affecting healthcare access, such as home safety, transportation, and other social determinants of health
Establish relationships with community partners and organizations to address member needs
Coordinate and participate in community events within the designated region and potentially across the state
Field Community Health Worker Requirements
Mandatory attendance for the first 90 days
Must have reliable transportation and a valid driver's license
Willingness to travel extensively (80-95%) within Georgia
High school diploma or GED required
Experience in community outreach, community health work, or social work
Strong organizational skills and time management abilities
Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint)
Field Community Health Worker Preferred Qualifications
Bilingual candidates preferred
Community Health Worker (CHW) certification is a plus
Experience in managed care and / or medical services
