Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using manual handling equipment like Hoyer lifts when necessary
- Monitor patients’ vital signs and observe for any changes in health status or behavior, documenting findings accurately in care plans
- Prepare nutritious meals tailored to individual dietary requirements and assist with feeding when needed
- Administer medications following prescribed schedules and ensure compliance with HIPAA regulations to protect patient confidentiality
- Support clients with cognitive impairments such as Alzheimer’s disease or dementia by implementing memory care strategies and providing engaging activities
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing living spaces
- Follow established care plans meticulously while collaborating with healthcare professionals to ensure optimal patient outcomes
Qualifications
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable
- Demonstrated ability to observe and report on patient conditions accurately and promptly
- Knowledge of HIPAA regulations and commitment to maintaining confidentiality at all times
- Experience with medication administration, vital signs monitoring, CPR/First Aid certification preferred
- Familiarity with specialized care such as Alzheimer’s/dementia care, autism support, neurodevelopmental disorders, intellectual disabilities, or hospice care is a plus
- Ability to perform manual handling tasks safely, including heavy lifting when required
- Strong communication skills and compassionate attitude toward supporting individuals with diverse needs
- Valid driver’s license and reliable transportation are often preferred for community-based visits
