Job purpose

We are seeking a professional, detail-oriented, compassionate Licensed Clinical Social Worker (Discharge Planner). To provide counseling services at the Military Medical Center (MOMMC), 99th Medical Treatment Facility (MTF), Nellis Air Force Base, Nevada. Contract for 8-koi: a diverse Florida-based service and consulting company that has been providing outing results for our clients for over 20 years. The contractor shall provide general support for medical operations.

Primary mission is to maintain medical readiness for worldwide contingencies by providing quality, cost-effective health care for Military Medical Center.

Duties and responsibilities

        • Interviews patients, family members, and reviews existing medical information to assess social and economic needs of patients and to arrange appropriate discharge plans.
        • Provides ongoing psycho-social assessment of patients' progress and needs.
        • Coordinates community resources to meet patients' social, physical, emotional and medical needs.
        • Determines need for advocating or intervention and documents intervention techniques and strategies.
        • Identifies problems which may impede discharge plan or patient's progress. Facilitates or coordinates the resolution of problems.
        • Assesses patients' financial status and eligibility for participation in government assistance programs. Coordinates referrals to appropriate Federal, state or county agencies.
        • Participates in reducing patients' lengths of stay by developing appropriate and safe discharge plans using a case management approach.
        • Provides an initial and on-going assessment of discharge plans.
        • Participates in multi-disciplinary team decision making.
        • Acts as liaison between patient and medical staff.
        • Provides grief and emotional support as needed to patients and families on an emergency basis.
        • Determines other interventions needed and coordinates intervention with outside of the agency.
        • Maintains detailed case files and compiles statistics on services provided.
        • Contributes to the efficiency and effectiveness of the unit's customers by offering suggestions and directing or participating as an active member of a work team.
        • Perform duties with consideration of age, growth/development, culture, emotional, spiritual and educational needs of the patient or other customer. This includes providing a safe environment and communicating effectively as well as integrating age specific requirements when performing procedures and providing care or other services.
        • Appropriately identifies and reports suspected adult and/or child abuse cases.
        • Utilizes and completes the rotation lists for DME, oxygen, home health, etc. and follows guidance for selection of vendor services (vendor rotation) when applicable.
        • Completes and submits transportation logs in a timely manner.
        • Passes face sheets/paperwork to co-workers in a timely manner.
        • Participates in the coordination of patient care transfers including acute care locally, out of state and out of country or to mental health / substance abuse facilities).
        • Assesses eligibility and refers patients to the appropriate agency for assistance or outpatient follow-up.
        • Attends and participates in unit specific patient care conferences.
        • Contributes to professional environment using appropriate leadership skills.
        • Appropriately identifies and refers patients in need of group home or nursing home.
        • Provides crisis intervention and counseling to patients.
        • Supports hospital-based specialty services (i.e. Endocrinology Services, Infusion Clinic and Renal Clinic.) through participation in assessment, counseling and committee participation specific to the needs of the specialty service.
        • Participates in education and/or in-service for the purpose of maintaining and improving competency to perform duties.
        • Responsible for the content of staff meetings whether through attendance (if mandatory and accessible) or review of minutes or other documentation.
        • Maintains licenses, certifications and/or registrations required by the position and/or organization. Provides required documentation in accordance with policy.
        • Identifies social, organizational, behavioral and interpersonal skills which would improve teamwork, organizational efficiency, enhance operational effectiveness and seek avenues for improvement.
        • Performs duties in a manner that efficiently utilizes supplies and other resources.
        • Interacts with patients, visitors, physicians, and co-workers in a manner that ensures patient rights and confidentiality/privacy at all times (including on the telephone and through the computer). Refrains from all public discussions regarding confidential/private information.
        • Performs duties in a manner consistent with the mission, vision, values, goals, and policies of the organization and department.
        • Assists in the orientation of and may function as a preceptor to employees and students new to the unit, as required.
        • Performs duties within the guidelines of corporate compliance regarding documentation, billing, charging, patient rights, and reporting.
        • Identifies patients at risk for injury (including near miss events) and suggests measures to appropriate management to prevent/reduce risk. Provides information regarding injury, risk, and/or4 protocols to co-workers.
        • Utilizes appropriate report6s and/or mechanisms (e.g. medication variance forms, employee accident forms, etc.) to identify system problems or noncompliance to standards for follow up by appropriate management.
        • Demonstrates knowledge of responsibilities and participates in the performance improvement processes as described in performance improvement plan. Participates in monitoring and TQM activities.
        • Assesses and reports and/or provides for correction of patient safety, environmental, and /or infection control issues.
        • Performs job duties within the established guidelines of Hospital policies to meet healthcare regulatory agency requirements and standards.
        • Contributes to the prevention of infectious diseases by performing duties within guidelines of Universal/Standard Precautions and provides guidance to patients/families and other staff.
        • Completes all responsibilities assigned associated with quality control activities (including but not limited to temperature logs, crash cart log. Point of Care testing and other department specific requirements).
        • Utilizes equipment in a safe and efficient manner (including recognition and resolution of equipment failures and related safety issues).
        • Demonstrates understanding of the organization’s fire and safety procedures as outlined in its policies and manuals; knows personal role in life saving activities (e.g. fire drills, evacuations procedures, internal/external disaster plans, code blue responses, infant abduction procedures).
        • Demonstrates an ability to prioritize and perform multiple tasks.
        • Demonstrates time management skills including using time productively during high and low volume periods.
        • Meets deadlines for assigned projects. Completes work assignment in a timely and dependable manner, meeting established deadlines.
        • Demonstrates accuracy and thoroughness in performing work assignments.
        • Demonstrates flexibility with work schedule changes; adjusts to the staffing needs of the Organization.
        • Accepts changes in job responsibilities as it relates to the classification and as required by organizational and departmental needs.
        • Performs effectively during high stress.
        • Adheres to organizational dress code; wears identification badge while on duty.
        • Greets and interacts with patients, families and other customers in a courteous and professional manner.
        • Listening effectively to patients, visitors and other customers, is responsive to their needs, and takes necessary steps to assist them.
        • Maintains a professional demeanor at all times.
        • Answers telephones promptly and courteously based on unit and Organizational policy.
        • Interacts with physicians and staff in a positive and professional manner.
        • Demonstrates respect, tact and diplomacy with all internal and external customers, whether in person or on the telephone.
        • Makes an effort to improve services and customer satisfaction based on knowledge gained through education, in-services, etc.
        • The contractor will abide by medical staff bylaws upon acceptance of privileges

Qualifications

Degree/Education:

  • Should be a graduate from a program accredited by the United States Council on Social Work Education (CSWE) and possess a Bachelor’s Degree in Social Work (BSW).

Experience:

  • Minimum of two years’ experience with at least one year in discharge planning/transitional care planning in a hospital setting that occurred within the last three years is required.
  • Ability to work in fast-paced clinical work environment. Able to document and maintain accurate and current notes in the patient electronic health record (EHR) and produce reports of evaluation and/or treatment.
  • Experience in Emergency Room, Inpatient Critical/Cardiac Care and Multi-Specialty Unit services required. Experience with crisis intervention, information and referral, knowledge of community resources, knowledge of discharge planning coordination to ensure a safe, comprehensive plan for placement post-ER/CCU/MSU and experience in working with multidisciplinary treatment teams.
  • Knowledge of local external payor sources (Medicare, Medicaid, TRICARE, Health Net, etc.); knowledge of payor source implications on discharge plan, resources and placement options to maximize patients' successful disposition.
  • Board Certification: Certification by the Association of Social Work Boards (ASWB) or the National Association of Social Workers (NASW) is desirable.
  • Licensure: Maintain an active, valid, current and unrestricted Social Workers License from any state or territory of the United States, the District of Columbia, Puerto Rico, or the U.S. Virgin Islands. It is the responsibility of the licensee to maintain a current and valid license without lapsing throughout the period of the contract. Any lapse will result in termination of contract.

Working conditions

  • Work Schedule: Shifts may vary from 8-, 10- or 12-hour shifts between the hours of 0630-1800 dependent on the mission needs not to exceed 80 scheduled hours in 2 weeks, which may include some weekends. For unexpected absences or illnesses, the Contract Worker will notify the clinic flight commander or designated representative as soon as possible, but no later than 2 hours prior to the start of the scheduled shift on the morning of the absence.
  • Duty Hours: Shifts may vary from 8-, 10- or 12-hour shifts between the hours of 0630-1800 dependent on the mission needs not to exceed 80 hours in 2 weeks, which may include some weekends.
  • On-Call Hours: On-call requirements for each labor category are as follows: N/A
  • Overage Hours (IAW Base Contract PWS): Overage hours are often used by the contractor to pay overtime: N/A
