Job Title: Community Outreach Coordinator
Reports to: Director of Community Outreach
BASIC PURPOSE
The Community Outreach Coordinator facilitates and organizes waiver applications for qualified
members of the community. The Community Outreach Coordinator develops relationships with
prospects and referral sources to assist with the Home and Community Based Services
application process and identifies additional business opportunities.
Community Outreach Coordinators are responsible for working & communicating with the PA
Independent Enrollment Broker (PAIEB), Area Agencies on Aging (AAA), Financial
institutions, and County Assistance Offices (CAO) to ensure all waiver applications are being
processed in a timely manner.
Community Outreach Coordinators are expected to establish and maintain professional
relationships with all referral sources, including but not limited to the following: Hospitals,
independent living facilities, assisted living facilities, sub-acute rehabilitation facilities, Senior
Centers, Senior housing facilities, County Assistance Offices, hospice organizations, service
coordination companies, area agencies on aging, and physicians.
PRIMARY DUTIES AND RESPONSIBILITIES
● Responsible for scheduling assessments for prospects with the PAIEB & Area Agencies
on Aging
● Conducting weekly follow up calls and maintaining strong relationships with prospects
on your case load
● Collecting and submitting required documentation to the PAIEB and CAO’s
● Develop and execute a marketing plan to meet or exceed monthly, quarterly, and annual
growth targets.
● Adheres to and promotes company policies and procedures
● Builds and cultivates relationships with industry contacts to gain industry knowledge,
referrals, and business development leads
● Ensures confidentiality of all consumer records and personnel files in accordance with
agency and HIPAA guidelines
● Ensures all services, functions, and employee practices are in accordance with
Affirmative Action policies and laws
● Responsible for ongoing compliance with all current federal, state, and local regulations,
company policies and procedures, and reimbursement guidelines
● Demonstrates a commitment to advancing 365 Health Services, LLC
● Must possess the personal characteristics of professionalism and commitment to high
standards with a strong work ethic, confidence, creativity, innovation, integrity, and
stability
● Performs other duties as necessary
MINIMUM REQUIREMENTS
● Bachelor’s Degree (preferred)
● Must meet all federal, state, and local requirements
● Excellent written and verbal communication skills
● Strong analytical skills
● Results driven, sense of urgency, and high standard of professionalism
