Job Summary
We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will possess a history working in pediatrics, strong clinical background, excellent communication skills, and a commitment to providing high-quality patient care. This role involves working in the patients home to ensure optimal health outcomes.

Duties

• Conduct thorough patient assessments and develop individualized care plans based on clinical findings.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
• Monitor patients' vital signs and report any significant changes to the healthcare team.
• Provide education and support to patients and their families regarding health management and treatment options.
• Collaborate with physicians and other healthcare professionals to coordinate patient care.
• Maintain accurate medical documentation in compliance with HIPAA regulations.
• Participate in quality improvement initiatives and adhere to NCQA standards.
• Assist in procedures such as IV insertion, catheterization, suturing, and wound care as needed.

Skills
The successful candidate should have experience or proficiency in the following areas:

• Medical office experience with knowledge of ICD-9/ICD-10 coding
• Knowledge of home health care practices
• Strong understanding of medical terminology, anatomy knowledge, physiology knowledge, and infection control practices
• GJ Tube feedings and medicine
• Seizure Monitoring
• Nurse will travel with mom for therapy and doctors appointments
• Experience with Hoyer lift.

Shifts: 28 hours a week broken down into shifts that are a couple of week days 10am-2pm(exact days to be decided by nurse) Weekends 11a-6p or 11a-7p
