Advanced Practice Provider 360X Staffing - Tucson, AZ $110,000 - $140,000 a year Title: Advanced Practice Provider Schedule: Hybrid in Tucson, AZ (Remote with in-home visits and periodic travel for training/meetings) Pay: $110,000–$140,000 annually plus performance-based bonuses Description/Responsibilities: Conduct comprehensive wellness exams in-home and virtually Educate patients and families on healthcare programs, treatment options, and preventive care Coordinate care with primary care providers, specialists, and ancillary services Prescribe medications, order diagnostic tests, and manage treatment plans Close gaps in care by ensuring timely screenings and diagnostic testing Participate in integrated care team meetings to develop individualized care plans Complete timely and accurate documentation in compliance with regulatory standards Deliver evidence-based, patient-centered care to reduce avoidable hospitalizations and improve outcomes Monitor patient progress and schedule follow-up care Maintain compliance with best practices and regulatory requirements Requirements: Active and unrestricted NP or PA license in the state of practice Board certification (NP: ANCC/AANP; PA: NCCPA) Current DEA certificate Minimum 2 years of direct patient care experience (managed care or chronic disease management preferred) Managed Care/IPA/Health Plan experience preferred Bilingual (English/Spanish) required Reliable transportation, valid driver’s license, and car insurance Proficiency with EMR systems, telehealth platforms, and data-driven patient management Strong communication skills and ability to work independently while collaborating with care teams To Apply: Download our app 360X JOBS, visit our website www.360xstaffing.com, and click "Join our Team".
