Overview
Now Hiring: Nurse Practitioners & Physician Assistants | Home-Based Primary Care, TCM & Wound Care
360 Medical Group | New Jersey · New York · Nevada · Illinois · Indiana | Mobile / Field-Based
At 360 Medical Group, we bring primary care to the patient instead of making the patient come to us. Our providers see homebound and medically complex patients in their homes, helping them stay healthy, safe, and out of the hospital.
We’re growing, and we’re looking for Nurse Practitioners and Physician Assistants who want to practice medicine that matters, with the flexibility of a field-based role.
What you’ll do
- Deliver home-based primary care to chronically ill, elderly, and homebound patients
- Provide Transitional Care Management (TCM) for patients recently discharged from hospitals and facilities, including medication reconciliation, follow-up, and care coordination to reduce readmissions
- Assess, treat, and manage chronic and complex wounds as part of our wound care program
- Use our assisted telehealth platform, where available and appropriate, to extend care between in-person visits
- Coordinate with home health agencies, specialists, pharmacies, and caregivers
- Complete timely, accurate documentation in our EMR
What we’re looking for
- Active, unrestricted NP or PA license in NJ, NY, NV, IL, or IN, or eligibility to obtain one
- Active DEA registration, or eligibility to obtain one
- NPs: national board certification (FNP or AGNP preferred; AGACNP considered)
- PAs: NCCPA certification
- Eligible for Medicare and commercial payor enrollment
- Valid driver’s license, reliable vehicle, and current auto insurance
- 1 years of clinical experience preferred. Home-based, geriatric, post-acute, or wound care experience is a strong plus. New grads with strong clinical rotations will be considered.
- Comfortable working independently in the field, with the support of a collaborative clinical team
- Wound care certification (WCC, CWS, CWCN) is a plus, and we offer training
Collaborative and supervising physician support is provided as each state requires.
Join us to deliver compassionate healthcare that transforms lives! We are committed to supporting your professional growth through ongoing training opportunities—including telehealth experience—and fostering a collaborative environment where your expertise makes a real difference in patients’ lives every day.
