Registered Nurse for ADHC

Job Summary

We are seeking a dedicated and compassionate Registered Nurse to join our Adult Day Health Care (ADHC) team. In this vital role, you will provide comprehensive nursing care to adults with complex medical needs, including those with developmental disabilities, memory care requirements, and chronic health conditions. Your expertise will ensure safe, effective, and person-centered care in a dynamic environment that promotes independence and well-being. As a key member of our multidisciplinary team, you will coordinate clinical services, monitor patient health, and support families and caregivers through education and advocacy. Responsibilities

• Conduct thorough patient assessments, including vital signs, physical examinations, and review of medical histories using electronic health record (EHR) systems such as Epic or Cerner.
• Administer medications and injections accurately, including IV infusions, immunizations, and botulinum toxin treatments for applicable conditions.
• Manage complex care plans for individuals with developmental disabilities, dementia (including Alzheimer's care), and chronic illnesses such as cardiac or respiratory conditions.
• Perform specialized procedures such as suturing, catheterization (including Foley and other urinary catheters), ostomy care, tube feeding management, and sterile dressing changes following aseptic techniques.
• Monitor patients' responses to treatments in various settings—from outpatient clinics to long-term care facilities—and adjust interventions accordingly.
• Collaborate with physicians, therapists, case managers, and other healthcare professionals to coordinate discharge planning, hospice & palliative care services, trauma medicine protocols, and emergency response procedures.
• Ensure compliance with HIPAA regulations and maintain meticulous medical documentation for all patient interactions in accordance with NCQA standards and ICD coding requirements (ICD-9/10).
• Provide triage assessments over the phone or in person to prioritize urgent needs for individuals with acute pain or infectious disease concerns.
• Support infection control practices within the facility by adhering to CDC guidelines and maintaining sterile processing standards.

Qualifications

• Valid registered nurse (RN) license in good standing with the state licensing board.
• Demonstrated experience working with individuals with disabilities, geriatrics (including memory care), or in behavioral health settings.
• Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with medical documentation standards is essential.
• Strong knowledge of physiology, anatomy, pharmacology related to medication administration and pain management.
• Critical care experience preferred—especially ICU or Level I/II trauma center backgrounds—and experience working in emergency medicine or hospital medicine environments.
• Skilled in performing complex procedures like IV insertion, phlebotomy, laboratory specimen collection/processing, ventilator management, and respiratory assessments.
• Excellent patient assessment skills combined with compassionate communication tailored for diverse populations including pediatrics (neonatal resuscitation procedures) and seniors requiring dementia or Alzheimer’s care.
• Ability to work independently while collaborating effectively within multidisciplinary teams; strong organizational skills for managing multiple priorities simultaneously.
