Part time Registered Nurse Case Manager
Anticipated Territory : Santa Clara County: San Jose*
Schedule: Saturday/Sunday
What you’ll do:
Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:
\n- Conduct Start of Care (SOC) Admissions - OASIS
- Conduct follow ups, evaluation visits as needed in designated territory
- Make home visits to patients in designated territory
- Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
- Monitor clients’ conditions, reporting changes to Clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
- Concierge for Case Managers: Dedicated resources assist with time consuming administrative tasks
- Flexible work schedules: Create your own schedule and hours
- Targeted territories. Typically patient visits are within a 5-15 mile radius.

- Current BLS/CPR Certification
- Valid CA Driver's License
- Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
- High level of clinical knowledge in home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
