What are the responsibilities and job description for the Admissions Coordinator position at 1ST SOURCE EMPLOYEE MANAGEMENT?
JOB SUMMARY:
The purpose of this position is to assist in the monitoring of established and maintained systems that
assure effective and efficient admission process in compliance with facility policies and procedures
within assigned facility region. The Lead Admissions Coordinator is in charge of overseeing and
appropriately administering the Admission process of residents into their assigned facility.
RESPONSIBILITIES AND MAJOR DUTIES:
1. Train, retrain, and/or monitor the assigned duties of the Admission Coordinator of the assigned
facilities at the direction of the Director of Admission.
2. Serve as a mentor and point of reference to the assigned Admission Coordinator within your
assigned region of facilities.
3. Interview potential residents and residents’ representatives to collect accurate initial data on
admissions inquiries.
4. Communicate admission inquiry data in written and verbal form to the nursing department and
administration in a timely manner.
5. Screen applicants for admission for proper placement in the facility.
6. Assign appropriate hospital numbers and room numbers to residents in consultation with the
nursing department and administration.
7. Greet and escort the new residents to the assigned nursing unit.
8. Identify potential residents whose needs can be met by the facility.
9. Ability to accept reservations for resident accommodations in an accurate and organized
manner.
10.Adhere to all admission, transfer and discharge policies and procedures.
11.Maintain and communicate an accurate facility census to all facility departments.
12.Keep accurate records of residents’ information; keep all residents’ information confidential.
13.Track and notify appropriate facility departments when a resident is admitted, transfers,
discharges or moves from one room to another.
14.Present, complete and carry out the facility admission agreement and all attachments for
potential and new residents and residents’ representatives.
15.Obtain required signatures on all admission forms.
16.Act as a liaison between transferring institutions and the facility; establish a good rapport with
transferring institutions.
17.Participate in marketing the facility and maintaining an acceptable census level.
18.Refer facility and resident concerns to appropriate individuals.
19.Observe infection control procedures.
20.Follow Residents’ Rights policies at all times.
21.Adhere to all facility policies & procedures and ensure they are adhered to by all responsible
parties and carry out disciplinary action for non-compliance as appropriate.
22.Ensure adherence to the Corporate Integrity Program and the Performance Expectations at all

23.Perform other related duties as directed by the Director of Admission and Customer
Resources, the Administrator and/or person of authority.
POSITION QUALIFICATIONS:
EDUCATION AND EXPERIENCE
High School Diploma or equivalent required. Associates Degree in Health Care, Business
Administration or related field preferred. Two (2) years’ experience in as an Admissions Coordinator
in a long term care setting required.
LICENSURE, REGISTRATION, COMPLIANCE CHECKS AND/OR CERTIFICATION
Successfully complete &/or comply with all required background checks to include but not limited to:
Ohio BCI&I fingerprint analysis, FBI (if applicable) fingerprint analysis, Nurse Aide Registry, HHS/OIG
list of Excluded Individuals/Entities (LEIE) & Ohio Medicaid Provider Exclusion and Suspension List.
RESPONSIBILITY FOR PATIENT CARE
Provide services based on physician orders. Communicate physician, patient and family needs with
the appropriate caregivers.
COMPUTER
Intermediate to advanced personal computer skills including electronic mail, routine database activity
and word processing is expected.
LEADERSHIP RESPONSIBILITIES
Responsible for the direction and management of the Admission process for the facility and should
display responsibility for the overall growth of the facility. Ability to set an example and provide
leadership and guidance to assigned Admissions Coordinators within the region.
RESPONSIBILITY FOR CONFIDENTIAL MATTERS
Works regularly with some information such as medical records or reports, financial reports,
information on computer screens or other electronic devices, and records that are restricted to
specific persons. Must comply with confidentiality policy.
