Physical Therapist (Full-Time)

We are seeking a dedicated and resident-focused Physical Therapist to support our Skilled Nursing, Assisted Living, and Memory Care populations within a campus-based healthcare community. This role offers the opportunity to provide continuous, person-centered care across the full continuum, allowing you to build meaningful relationships and support long-term outcomes for residents.

Position Overview
The Physical Therapist delivers individualized therapy services to residents, supporting mobility, functional independence, and overall quality of life. This role works closely with an interdisciplinary clinical team to ensure comprehensive, coordinated care tailored to each resident’s needs and preferences.

Key Responsibilities
• Provide one-on-one physical therapy services to residents across Skilled Nursing, Assisted Living, and Memory Care settings
• Develop, implement, and adjust individualized treatment plans based on resident goals and clinical needs
• Support residents in improving mobility, strength, balance, and functional independence
• Deliver care using a holistic, resident-centered approach that addresses physical, emotional, and overall well-being
• Collaborate with interdisciplinary teams to ensure continuity and quality of care
• Accurately complete clinical documentation in compliance with regulatory and reimbursement requirements
• Participate in care planning and contribute to positive resident outcomes across the continuum

Qualifications
• Current Physical Therapist license in the State of Maryland
• Must meet all applicable Medicare and state regulatory requirements
• Current CPR certification
• Experience completing clinical documentation in accordance with reimbursement standards
• Experience working with older adult populations across multiple diagnostic groups preferred
