Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The successful candidate will provide high-quality patient care in a dynamic and fast-paced environment. The ideal candidate will have experience in home healthcare and working with the geriatric population.
Scheduling needs:
- Monday: 12hr shift day and night available
- Wednesday: 6am to 8pm shift
- Friday: 12hr shift day and night available
- Saturday: 8am to 8pm
- Sunday: 9pm to 8am
Responsibilities:
- Provide direct patient care, including administering medications, performing wound care, and assisting with treatments.
- Conduct physical examinations and document patient vital signs, medical histories, and observations.
- Utilize electronic health records (EHRs) to update patient information and track treatment progress.
- Collaborate with healthcare providers to develop and implement patient care plans.
- Provide education and support to patients and their families regarding their condition, treatment options, and prevention strategies.
- Maintain a safe and clean environment, adhering to infection control protocols and other safety standards.
- Monitor and report patient progress, including any changes or concerns.
Experience:
- A current LPN/RN license in the state of employment.
- Experience working in home healthcare settings.
- Familiarity with EHR systems.
- Strong communication and interpersonal skills.
- Ability to maintain confidentiality and adhere to HIPAA guidelines.
- Experience with gtube/trach/vent highly preferred
- Experience with patients with seizures highly preferred
