Position Details:
- Locations: Patients' homes in Jersey City and/or Lebanon, NJ
- Pay: $42.00 - $54.00 per hour
Key Responsibilities:
- Deliver high-quality nursing care to patients in their homes.
- Conduct comprehensive health assessments and develop personalized care plans.
- Administer medications and treatments as prescribed.
- Monitor patients' progress and adjust care plans as needed.
- Educate patients and families on condition management and wellness strategies.
- Collaborate with other healthcare professionals for coordinated care.
- Maintain accurate and timely documentation of care activities.
Experience & Skills Required:
- At least 2 years of nursing experience in a home health setting.
- Expertise in trach and vent care and tube feeding management.
- Experience working with children and families.
