Job Summary:
As an LPN in our home healthcare team, you will be responsible for delivering comprehensive nursing care during the day shift. You will work closely with the patient and their family, providing support and medical care in a home setting.
Key Responsibilities:
- Provide high-quality nursing care to a wheelchair-bound patient in their home.
- Administer medications and treatments as prescribed by the patient's physician.
- Monitor the patient’s vital signs, manage medical equipment, and assess overall health status.
- Assist with activities of daily living and provide support with mobility and positioning.
- Collaborate with the patient's family to ensure effective care and comfort.
- Document all care activities, observations, and changes in the patient’s condition accurately.
- Respond promptly to any medical emergencies or changes in the patient’s condition.
Qualifications:
- Valid and active LPN license in the state of New Jersey.
- g-tube experience necessary.
- Strong clinical skills and the ability to work independently.
- Excellent communication skills and a compassionate approach to patient care.
- Reliable, with a strong sense of responsibility and professionalism.
- Flexibility to work day shifts as required.
