Position Summary

The Behavioral Health (BH) Clinic Care Coordinator provides administrative support to the outpatient clinics in DC & Maryland within Mary's Center's Department of Behavioral Health. The BH Clinic Care Coordinator is responsible for organizing, coordinating, and monitoring all administrative tasks for Behavioral Health participants including management and facilitation of internal and external referrals, reviewing patient records, completing chart audits, and triaging calls. This includes following up with participants who have been hospitalized, facilitating transfers, and providing connecting resources post-discharge. The BH Clinic Care Coordinator supports program expansion opportunities, creates the necessary tools/applications/letters needed to support successful clinical partnerships, and engages in the necessary outreach activities with the support and guidance of the Clinic Director to achieve those goals.

Compensation: Salary Range $45,000-$55,000

Reportability

This position will report directly to the Behavioral Health Clinic Director.

Essential Duties and Responsibilities

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Programmatic/ Leadership Responsibilities:

  • Identify, compile, and maintain a database for special programs and services for participants: neuropsychological evaluations for people without insurance to cover it, PHP/IOPs for particular disorders or age-groups (e.g., eating disorders, SUD, neurodevelopmental needs beyond the routine / generalist level, OCD).
  • Assist with submitting/processing internal and external referrals with awareness of external programs' referral processes.
  • Keep an up-to-date list of insurances accepted for resources identified.
  • Build relationships/partnerships with external organizations to streamline referrals.
  • Follow up after emergency hospitalization to find out which hospital has accepted our participant.
  • Coordinate with hospitals for in-patient admission and for discharge planning and summaries when participants are admitted to a higher level of care.
  • Maintain annotated lists of the resources for clinicians to access.
  • Coordinate with CSWs to support non-participant-specific dissemination of resources such as special housing programs.
  • Assist participants who require additional support with insurance renewals.
  • Provide participants with training on Zoom access.
  • Frequently monitor CRISP data and work with health informatics team when a Behavioral Health participant has been hospitalized.
  • Coordinate and build partnerships with day programs to identify specialized services.
  • Provide Hospital/ER follow up with Mary's Center participants.
  • Attend weekly staff meetings for both clinics and report out on resources.
  • Ensure providers understand and follow all policies, procedures, and workflows related to care coordination.
  • Participate in quality improvement process, implement systems to gather outcome measures, maintain and provide statistics, as necessary, and provide monthly and quarterly reports to the Vice President of Behavioral Health and others as indicated.
  • Participate in Mary's Center committees and initiatives as assigned.
  • Complete regular chart audits to ensure clinical compliance.
  • Ensure providers are aware of new resources by sending weekly updates to the department.
  • Other duties as assigned.

Qualifications

Knowledge, Skills & Abilities

  • Requires a bachelor's degree.
  • Must have two to five years of care coordination experience.
  • A thorough understanding and knowledge of patient confidentiality issues and HIPAA compliance/regulations are essential.
  • Ability to apply common sense understanding to carry out highly-complex, multi-step instructions and make appropriate independent decisions as necessary.

Language Requirement - Ability to communicate effectively in English is required. Additional language proficiency or fluency preferred.

Physical Demands - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.

Work Environment - This is a hybrid position - remote and onsite, as needed. Mostly in a typical office or clinic setting with quiet to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions.
