CAMBA is Hiring a Community Health Worker, United Health Foundation Infant and Maternal Health Program Near Brooklyn, NY

Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City. CAMBA serves more than 65,000 individuals and families each year, citywide, including almost 13,000 youth. Our 180 programs in over 100 locations improve the lives of a diverse cross section of New Yorkers. From homelessness prevention in Staten Island to supportive housing in the Bronx; from employment training in Manhattan to after school programs and college access in Brooklyn; from family shelter and support in Queens to increasing affordable housing across the city, CAMBA provides holistic services to help struggling New Yorkers stabilize their lives and become self-sufficient.
CAMBA's United Health Foundation Infant and Maternal Health Program will consist of imbedded and continuous services provided to the communities of our two Brooklyn-based family shelters and the nine NYCHA Public Housing Developments served by CAMBA's Cornerstone Community Centers. We anticipate serving approximately 500 women annually over the three-year grant period.
Position: Community Health Worker
Reports To: Community Health Supervisor
Location: 21 Snyder Avenue, Brooklyn, NY 11226
What The Community Health Worker Does:
  • Conduct outreach to identify high need women and infants from events at community venues, schools, hospitals, clinics, churches, shelters, substance abuse treatment clinics, emergency rooms etc.
  • Plan and utilize creative outreach strategy events (mailings, home visits, presentations, workshops, door-to-door flyer distribution) to motivate participants to agree to use UHF program services.
  • Plan and facilitate educational presentations to participants, providers and others regarding UHF and a variety of topics.
  • Conduct initial intake and/or assessment of participants and family situations and needs, and periodic re-assessments.
  • Assist participants in completing all CAMBA intake applications and consent forms.
  • Create and maintain participant files.
  • Manage a small caseload across the life cycle.
  • In collaboration with participants, prepare initial and periodic revisions of service plans, including short-term and long-term goals.
  • Assist participants in attaining their goals by identifying and locating community resources for participants and by making referrals to appropriate services both within and outside CAMBA (i.e., physicians and dentists who can assure appropriate care throughout key life stages and support services like WIC and food stamps).
  • Work with participants to overcome barriers/goals, assist participants in advocating for themselves and in moving toward self-sufficiency.
  • Monitor participants' progress toward their goals and document via progress notes.
  • Act as liaison and advocate for participants with referral sources regarding participant's progress related to their education, healthcare, housing issues, etc.
  • Follow-up with participants for a period of time after successful completion of their primary goals to assure their stability.
  • Escort participants to appointments (educational, medical, social service, etc.) and act as translator if necessary.
  • Assist participants in completing applications (i.e., assist with gathering documentation) for benefits and entitlements (i.e., health insurance), process applications on participants' behalf and follow-up to ensure enrollment.
  • Provide all required data and narrative information for weekly/monthly/quarterly/annual reports in a timely manner.
Minimum Education/Experience Required:
  • High School diploma or GED and 4 years of applicable experience and/or equivalent experience
Other Requirements:
  • Some evenings and/or weekends may be required.
Compensation : $35,000-$38,000 annually
When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.
Status: Full-time (35 hours per week)
