We are seeking a compassionate and experienced Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for coordinating and providing comprehensive patient care, ensuring the delivery of high-quality services to our patients. Responsibilities:
- Conduct assessments and develop individualized care plans for patients
- Coordinate and collaborate with interdisciplinary healthcare team members to ensure effective patient care
- Monitor patient progress and make necessary adjustments to care plans
- Provide education and support to patients and their families
- Advocate for patient needs and ensure their rights are respected
- Maintain accurate and up-to-date documentation of patient interactions and care provided
- Adhere to medical protocols, standards, and guidelines
- Collaborate with community resources to facilitate patient access to necessary services
Experience:
- Valid Registered Nurse license
- Minimum of 2 years of nursing experience in acute care or primary care setting
- Experience in case management or care coordination preferred
- Knowledge of medical terminology, HIPAA regulations, and managed care principles
- Strong assessment, communication, and organizational skills
- Ability to work independently as well as part of a team
- Experience working with diverse populations, including pediatrics and behavioral health patients
- Proficient computer skills
