The Physical Therapy Assistant (PTA) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Physical Therapist (PT) or Director of Nursing DON.
PTA to service the Ocala, Florida area
Duties and Responsibilities:
•Assists Physical Therapists (PT's) and provides maintenance care
•Implements physical therapy treatment program
•Assists Therapists and other PTA's in overall treatment plan
•Provides maintenance care to patients per Case Conference and the patient plan of care
•Prepares patients for treatments
•Responsible for routine cleaning and maintenance of department equipment and supplies
•Documents pertinent patient/treatment activities per Secure Care policies
•Understands and adheres to established Secure Care policies and procedures
•Provides physician prescribed physical therapy under a plan of care established by the Director of Nursing (DON) and/or Physical Therapist (PT)
•Improves or minimizes residual physical disabilities of the patient
•Returns the individual to optimum and productive level within the patient's capabilities
•Participates with all other health care personnel in patient care planning
•Performs all skilled procedures as ordered by physician and according to the plan of care established by the Director of Nursing (DON) and/or PT
•Consults with Director of Nursing (DON) and/or Physical Therapist (PT) regarding change in treatment
•Instructs patients and family members on in-home programs and activities of daily living
•Participates in in-service programs and presents in-service programs as assigned
•Participates in QAPI activities as assigned
•Attends all Case Conferences, as scheduled
•Prepares medical records, progress notes, and updates care plans for each patient visit in a timely manner as per Secure Care policy
•Performs services planned, delegated, and supervised by the Director of Nursing (DON) or Physical Therapist (PT)
•Assists in preparing clinical and progress notes
•Participates in educating the patient and family
•Participate in organizational committees as identified that enhance the working relationships between departments within Alivia Care, Inc.
•At all times, represent Alivia Care, Inc. professionally through care and/or services provided to all clients and patients
•Comply with Alivia Care, Inc. policies, procedures, and standard practices
•Maintain the confidentiality of patients and families within Alivia Care, Inc. as per government mandates
•Use resources in a fiscally responsible manner
•Promote Alivia Care, Inc. through participation in community and professional organizations
•Improve own professional knowledge and skill level
•Additional duties, as assigned
