The Marti Health RN Nurse case manager and care coordinator serves as the leader of a team that supports patient-centered case management and care coordination for patients and their caregivers. The nurse manager provides age-appropriate, culturally sensitive care, maintains a safe environment, educates patients and their families about healthy practices and treatment modalities, assures continuity of care, coordinates care across settings and among caregivers, manages information, communicates effectively, leveraging the Marti Health technology for efficiency. The position utilizes the nursing care process to provide and plan case management and overall care, i.e. assessment, nurse diagnosis, planning and coordination, implementation, and evaluation. The selected candidate advocates for clinically competent and culturally sensitive care, participates in performance improvement, leadership, problem solving, delegation, and conflict resolution.
This position seeks to assure the necessary level of care for medical requests and needs, avoiding unnecessary ED admissions and hospital care. Key manager of patient portal messages, even sometimes from community health workers or intended for physicians. Triages and manages patient messages and lab triage, determining level of medical necessity and urgency, and appropriateness of and level of medical response. In particular, coordinates optimal medication therapeutic management and determinations regarding necessity of urgent versus non-urgent care using evidence-based guidelines. The selected candidate will assist with prior authorizations, utilization requests, referrals for specialty services, assists with addressing health related social needs and performs various other responsibilities as deemed appropriate for patient care and safety.
Requirements
Responsibilities:
  • Conduct outpatient case management to include:
    • Conduct in-person and telephone visits
    • Manage patient portal, community health workers or physicians directed messages.
    • Triages and manages patient messages and lab triage.
    • Determines level of medical necessity and urgency of telephone or text requests
    • Determine appropriateness of and level of medical response to patient phone and other requests.
    • In particular, determines needs for medication refills and necessity of urgent versus non-urgent response.
    • For medication requests, assists with prior authorizations.
    • For utilization requests, determines need for referrals for specialty appointments, and assists with appointments.
    • Performs various other responsibilities as deemed appropriate for patient care and safety.
  • Under Advanced patient practitioner (APP) or physician supervision, electronically propose prescription refills to be confirmed by APP or physicians.
  • Troubleshoot prescription refill issues.
  • Assess pain (acute and chronic)
  • Refer to a 24/7/365 on-call provider schedule, fielding calls from outside providers, patients, and institutions about medical home sickle cell patients.
  • Participate in daily case management huddles.
  • As needed, supervise other nurse or community health worker team members, and train them in case management.
Minimum Requirements:
  • A Bachelor of Science in Nursing is required. Advanced degree is desirable.
  • One (1) or more years' experience as a Registered Nurse required. Far more is strongly preferred.
  • Ambulatory and inpatient care experience in sickle cell disease is highly desirable.
  • Ability to demonstrate empathy, cultural sensitivity and provide compassionate care is a prerequisite.
  • Previous case management experience and team experience in managed care is strongly preferred.
  • 1-3 years supervisory experience desirable.
  • Specific area of previous specialty experience preferred.
  • Current state license as a Registered Professional Nurse required
  • Current American Heart Association (AHA) Basic Life Support (BLS) health care provider card required
