The Family Development Specialist - Clinic Services, serves in a role ensuring the quality and efficiency of healthcare services in a clinical setting. Their primary responsibility is to support the clinic staff, healthcare providers, and management in maintaining high standards of care, patient safety, and education for low-income individuals. Must work effectively and efficiently with physicians, staff, patients, public, and external agencies.

• Records Social Determinants of Health (SDoH)for each patient.
• Reviews SDoH and Patient Health Questionnaire (PHQ-9) assessments.
• Utilizes Family Strengths approach to provide Family Development and goal coaching according to the SUCCESS model upon patient
• Performs all Referral Coordination as needs are identified.
• Collect and analyze data related to quality performance and indicators, reporting to the Agency and clinic leadership upon
• Collaborate with providers, nurses, and administrative staff to implement quality improvement strategies upon
• Participate and implement clinic-specific procedures and
• Ensures all Quality measures are followed, providing job studies and efficiencies for
• Provides monthly reports to Management for needs assessments as developed with patients.
• Aids with clinical functions or front desk as needed including, but not limited to: prior authorizations, referral coordination, normal labs, front desk
• Other duties as assigned.

An ideal candidate will have an employment background in medical services (example: CNA) as well as social services. Possess excellent oral and written communication skills. They must have an active and unhindered Driver’s License. Successfully pass Criminal Background including FBI fingerprint and Child Maltreatment checks. Must be willing to obtain a Family Development Certification, the agency covers the education expense for this certification. 

Outstanding candidates will also have these preferred qualifications: Post-secondary education with emphasis in Social Sciences. Soft skills; social perceptiveness, active listening, critical thinking, empathy, collaboration, organization, and time management skills.

PHYSICAL REQUIREMENTS

Occasional lifting and moving of general office equipment, furniture, and project materials. Long periods of sitting, reading, keyboarding alternating with long periods of standing. Frequent driving within the 6-county service area with occasional out-of-area functions/training.
