CoCM Behavior Health - LPN

Collaborative Care Case Management for Behavior Health

Empowering Healthcare Services is seeking an experienced CoCM with an LPN license to join our team of Mental Health professionals. We strive for top talent and seek a talented, caring professional with broad knowledge of Case Management.

As a CoCM, you will be a core member of our collaborative mental health team approach at Empowering Healthcare Services. The CoCM will support and coordinate mental health care on an assigned patient caseload. The client focus is geriatric patients residing in an inbound setting and/or skilled nursing facility. There will be times of minimal travel within the Atlanta region.

Objectives:

• Screen and assess patients for everyday mental health and substance abuse disorders
• Support the mental and physical health care of patients on an assigned patient caseload.
• Facilitate patient engagement and follow-up care.
• Provide patient education about everyday mental health and substance abuse disorders and the available treatment options
• Systematically track treatment response and monitor patients' changes in clinical symptoms and treatment side effects or complications
• Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment
• Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate
• Provide or facilitate problem-solving treatment or behavioral activation as clinically indicated
• Participate in regularly scheduled caseload consultation
• Document patient progress and treatment recommendations in required systems
• Facilitate treatment plan changes for patients

Required Skills:

• Excellent written and verbal communication skills
• Must possess maturity, self-confidence, objectivity, and a positive attitude
• Self-directed with the ability to function well under stress, handle change, and function in a fast-paced environment
• Robust assessment, interview, organizational and problem-solving skills
• Knowledge regarding local, state, and federal regulations is required
• Knowledge of community and state-wide resources and programs
• Ability to work collaboratively with physicians, members of the care team, and the patient/family to assist through the continuum of care

Experience:

• Nursing: 3 years
• Case Management: 3 years
• Mental Health: 1 years (preferred)

License/Certification:

• Must hold an LPN license in the State of Georgia

Job Type: Full-time

Salary: $23.95 - $28.50 per hour

Expected hours: 40 per week
