COPE Community Services is Hiring a Discharge Planner Near Tucson, AZ

We are looking for a Discharge Planner to join the QM Department. Under supervision of the Program Director and/or designee, is responsible for encouraging and empowering COPE members to recover by providing services which will assist COPE members to more effectively utilize the outpatient behavioral and medical health system and/or understand and cope with the stressors of the member’s disability. A particular focus will be on quality discharge planning for members in hospitals or other facilities.

Minimum Qualifications:

  • Bachelors or master’s degree, no experience necessary.
  • Associate degree plus two (2) years behavioral health work experience.
  • High School Diploma or GED plus Peer Support Specialist certification plus three (3) years of behavioral health work experience.
  • High School Diploma or GED plus Medical Assistant certification plus three (3) years of behavioral health work experience.
  • High School Diploma or GED plus four (4) years of behavioral health work experience.

Preferred Qualifications:

  • At least one-year experience working as a Recovery Coach at COPE.
