Head Injury Association has immediate openings for Community Habilitation Specialists!
The Community Habilitation Program provides one-to-one individual, customized support and services to Traumatic Brain Injury Survivors and individuals with Intellectual Disabilities.
The goal of this program is to develop life management skills as well as community integration. The supports provided are based on individual needs and are person-centered. These services include, shopping, recreation and self-advocacy. The frequency and duration of these services are specific to each individual’s needs.
Essential Job Responsibilities
· Ensuring safety and providing appropriate supervision of individuals supported
· Building positive relationships and being a positive role-model and companion to the individuals supported
· Using effective written and verbal communication
· Adhering to HIA’s policies, procedures and regulations
· Transporting individuals to recreational activities and community outings such has the movies, parks and local fairs
· Helping the individuals achieve their goals and improve their quality of life
Available Shifts:
Morning and Evening Shifts Available
· Part Time and Per Diem openings
Education and Experience:
· High school diploma or GED required; college degree preferred.
· Previous experience working with OPWDD or TBI individual's a plus
· Must have a valid NYS license
· Excellent written and verbal communication
· Training offered in-house
