JOB STATEMENT/SUMMARY

The behavioral health case manager provides behavioral health case management services for patients at Coplin Health Systems including but not limited to patient education, linkage and referral.

ESSENTIAL FUNCTIONS & RESPONSIBILITIES OF THE POSITION

· Is accessible to Coplin Health systems clinical staff members during clinic hours to facilitate patient care.

· Assesses clients’ physical and mental wellness, needs, preferences and abilities and using these to assist the behavioral health therapist when developing tailored care plans

· Working with clients, family and friend support networks and health care professionals to put care plans in place

· Listens to clients’ concerns and providing intervention as required

· Records clients’ progress including charting referrals, home visits and other notable interactions

· Evaluates clients’ progress periodically and makes adjustments as needed to improve outcomes

· Follows up with discharged clients to ensure they were satisfied with services and that they are still in good physical and mental health

· Conducts behavioral health triage activities when presented with patients in crisis and utilizes the CHS Crisis Intervention Handbook to determine the appropriate course of action.

· Performs all duties from a culturally sensitive framework.

· Ensures education materials related to behavioral health services are available to patients.

· Assists behavioral health providers in group therapy sessions.

· Provides requested interventions related to chronic disease management.

· Coordinates relationship with Drug Courts in Coplin’s service counties.

· Provides patients with linkage and referrals to agencies that can help them meet basic needs including access to food, shelter, clothing, and benefits for improved outcomes.

· Maintains up-to-date contact information about commonly used community resources.

· Represents primary care behavioral health program in meetings with community organizations, government groups, and social agencies, updating those entities on the activities and goals of the program.

· Performs other duties as assigned by the Behavioral Health Director

JOB REQUIREMENTS AND PREFERRED QUALIFICATIONS

Education (level and type) and Related Work Experience

Candidates with a bachelor’s degree in social sciences or 3 years related experience in the behavioral health field as well as experience with an electronic health record are strongly preferred. [ML1]

This position is described as light physical activity job. Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.

This job operates in a professional office environment. This role routinely uses standard office equipment such as laptop computers and smartphones. The noise level in the work environment is usually moderate with frequent interruptions.
