FULL JOB DESCRIPTION

Cordial Healthcare Providers is seeking to hire a part-time Nurse Practitioner for our growing Mobile Medical practice. Cordial Healthcare Providers is a team of Board-certified Doctors and Nurse Practitioners who provide primary care to medically complex, chronically ill, and homebound patients. We visit patients at home, Assisted living and group homes for home health evaluation and to provide their health care needs.

This NP position offers flexibility where you can make your own schedule to visit and care for our patients in San Fernando Valley and Los Angeles area.

DUTIES AND RESPONSIBILITIES:

• To visit patients at home, Assisted living and group homes for Home Health evaluation and referral as deemed necessary.
• Primary Care-acute and chronic disease management.
• Perform physical examination and annual wellness visit.
• Provide accurate assessment, diagnosis, and necessary treatment consistent with standard of care.
• Provide comprehensive and accurate documentation.
• Orders and interprets laboratory and diagnostic tests to identify and analyze patient's medical problems and health care needs.
• Educate patients and their family members/caregivers on acute and chronic diseases, health promotion, health prevention, health maintenance, Plan of care and importance of compliance.
• Create treatment plan for acute and chronic diseases based on patient’s complaint/s, condition and findings.
• Medication evaluation and management which includes teaching, prescriptions and refills. Prescribe medications based on evidence-based and treatment guidelines.
• Post-hospital follow-up visit.
• Works independently in a professional, timely and competent manner with minimal supervision.

QUALIFICATIONS

• Graduate of an accredited Nurse Practitioner program
• Board Certified through AANP or ANCC
• Holds an unrestricted California License and certification
• We prefer NP who is bilingual in Armenian and Farsi
• Compliant with California Board of Registered Nursing and DEA regulations.
• With valid DEA and NPI numbers
• BLS and ACLS
• PECOS registered preferred but not required
• Must have a valid driver’s license with car liability insurance
• Previous experience in Primary Care, Hospital, ER, Urgent Care is preferred but not required.
• Excellent interpersonal and communication skills.

Job Type: Full-time

Pay: $100.00 - $160.00 per hour
