Corbin Health & Rehabilitation Center is Hiring a HC - LPN / RN - Clinical Coordinator/Unit Coordinator Near Corbin, KY
Essential Duties & Responsibilities
To perform this job successfully, an individual must be able to perform each essential duty and responsibility satisfactorily.
Complete all admission orders if possible, if floor nurse completes after hours, Clinical Coordinator must review the next day for accuracy.
Check to ensure order was noted off correctly and put on MAR, Treatment Sheet, NA Flow Sheet, Lab Book, etc.
Place the orders on the Care Plan that day.
Put the information on the Kardex for the NA’s to know how to care for the resident.
Do RD referral if indicated.
Check report for completion and accuracy
Reports of falls, check to see if 72 hour fall follow-up was initiated.
Makes sure a new fall intervention was put in place to address the causative factor of the fall. Puts the new intervention on the Care Plan, Kardex, and NA Flow sheet.
Checks for the need to referral to PT/OT.
Ensures all lab results from the previous day are received.
Calls MD or fax results to MD.
Marks/Highlights off of lab book as results are received.
Pull the pink requisition copy from the lab folder as results are reported and filed.
Does RD referrals in indicated.
Reviews Treatment Books to ensure skin assessments are done for the previous day per schedule.
Review skin assessments to make sure measurements are documented on the back of the assessments.
Ensures treatment is written according to protocol for all new wounds.
Ensures treatments are changed every 2-4 weeks if no improvement.
Visualize/reviews all wounds as least weekly PRN.
Writes orders for RD recommendations the same day that recommendations are received.
Do RD referrals for labs, new wounds, worsening wounds, being below Estimated Fluid Needs, poor food intake, weight gain/loss, etc.
Review FISH List daily.
Review the staffing schedule daily.
Assist with coverage.
Listens to staffs’ concerns.
Report lack of staff resolution/actions taken.
Informs staff of any changes.
Assist with in-services.
Required to make sure residents are turned and positioned properly.
Appropriate Skin Preventative measures are in place, such as pressure relieving cushions, heel floaters, pillows under head, etc.
Fall preventive measures are in place such as a floor mat, low bed, toileting plan, non-skid footwear, etc.
Oxygen on the correct liter flow and on the resident.
Foley catheters-bags covered, not on the floor, anchored with tape or Cath Secure, etc.
Oral care, nail care, hair clean and combed, dressed appropriately.
Water pitcher or Thickened Liquid Cooler in reach and being used.
Report any abuse/neglect allegations to Adm/DON immediately.
