Shift : Days / 8a-4:30p, 8x5, 40 hrs per week, M-F, weekends per rotation
Length : 12 Weeks
Weekend rotation (currently every 5rd weekend, but could change), case load of ~13 patients, may need to cover a holiday this term
Requirements:
- OH License
- 2 years experience
- BLS
- 3 years home hospice experience preferred
JOB SUMMARYIs responsible for making home visits to assigned patients requiring skilled intermittent professional nursing services in the home. All findings and/or changes regarding patient are communicated to the following physician.
MINIMUM QUALIFICATIONSMinimum Qualifications include: Valid state driver’s license, Minimum of eighteen (18) years old, Have an acceptable Motor Vehicle Report (MVR), Must provide proof of insurance, if applicable - Associate's Degreein Nursing. Current RN licensure in the state of Ohio
3 years home hospice experience preferred
