The RN is a key component of our IDT, who manages the care to provide the best quality services to our patients and their loved ones wherever they call home. *PRINCIPAL RESPONSIBILITIES (includes but not limited): * ✓ Initial complete assessment to determine clinical appropriateness, level of care and diagnosis ✓ Help patients and family to understand needs and services ✓ Develop initial comprehensive assessment to establish plan of care and revision of care plan as indicated ✓ Provides professional nursing care by utilizing all elements of nursing process ✓ Administer medications and treatments as prescribed by the physician as directed by the plan of care plan of care. ✓ Communicate and collaborate with IDT including patient and family throughout the entire planning process ✓ Initiates appropriate preventative and palliative nursing procedures ✓ Communicate with the physician regarding patient's needs and reports any changes in the patient's condition ✓ Coordinate with office for needed resources, disciplines, pharmacy, supplies and/or DME in a responsible manner according to need of patient. ✓ Oversee care of LVN’s and HA *JOB REQUIREMENTS: * ✓ Current unrestricted California RN license ✓ Min 1-year RN nursing experience in SNF, Acute Care or Hospice/HH ✓ Valid CPR certification ✓ Reliable Transportation ✓ Valid California DL ✓ California vehicle insurance ✓ Understands the Hospice philosophy and principles of death/dying. ✓ Able to be a self-starter working under minimal supervision ✓ Superior customer service skills ✓ Excellent problem-solving skills ✓ Attention to detail and observation/assessment skills ✓ Good time management skills ✓ Must be organized ✓ Strong communication skills ✓ Strong writing skills ✓ Must be willing to travel *WE ARE PROUD TO OFFER COMPENSATION/BENEFITS: * ✓ Competitive Salary ✓ Flexible Schedule ✓ Phone reimbursement ✓ Health, Vision, Dental ✓ Opportunities to advance and grow your career Equal Opportunity Employer
