Responsibilities
Position Summary:
Incumbent coordinates and facilitates transfers of patients from the facility to home health agencies. Incumbent assumes responsibility and accountability for coordination and planning prescribed home care services equipment and family support mechanisms required to enable a patient to transition from the hospital to home while still in need of skilled care. Incumbent assesses all referrals for appropriateness of home health services utilizing agency objectives policies and procedures. Incumbent also assists with quality assurance/quality improvement and utilization review activities.
Qualifications
Minimum Qualifications:
Verbal and written communication skills and computer skills required
Preferred Qualifications:
Recent experience in a related field preferred
Knowledge regarding discharge planning preferred
Pay Range
$29.36 - $32.84 /hour
