Position Overview

The Quality Assurance Investigator is responsible for conducting investigations involving consumers/employees in the DD, BH, HS, TS and VT Divisions based on incidents/special issues.

  • SCOPE OF ROLE:
  • The Support Services functions at S:US are carried out by a team of professionals with differing backgrounds who share deeply in the S:US mission. We provide resources and support to the program departments to collectively pursue that mission. Support Services is a collegial, supportive and dynamic culture that continuously seeks to improve.     As a QA Investigator, you will be joining a dynamic department that protects the organization as well as the people we serve. Specifically, your role will be to objectively investigate incidents and alleged misconduct involving individuals served by S:US, as well as S:US employees and contractors. You will be responsible for conducting investigations in all of our service delivery areas, including Developmental Disabilities, Behavioral Health, and Homeless Services. You will be expected to interview all relevant parties, review documentation, analyze all of the pertinent information, and then write a well thought out, thorough and organized investigation report, all while meeting regulatory time frames for the completion of investigation reports. You will also ensure adherence to the S:US mission, core values, and code of ethics.

  • ESSENTIAL DUTIES & RESPONSIBILITIES:
  • Investigation Activities

    • Conduct investigations involving individuals served by S:US, as well as S:US employees and contractors, in all of our service delivery areas, including Developmental Disabilities, Behavioral Health, and Homeless Services.
    • Schedule and conduct interviews with all relevant parties.
    • Review and analyze various reports and documents pertinent to the investigation, and follow up on missing pertinent information
    • Complete written investigation reports as per established protocols and regulations.
    • Participate in arbitration or court hearings based on investigations when required

    Audits/Reviews

    • Participate in internal audit reviews at various programs.
    • Conduct site visits to ensure compliance with all regulatory mandates.
    • Conduct follow up audits to confirm corrective actions have been implemented and are effective.
    • Draft written reports summarizing findings of various audits/reviews.
    • Help to develop audit tools to be used in various audits/reviews.

    Trainings

    • Conduct Incident Management/Investigation trainings for staff.
    • Help to develop and update training materials for Incident Management/Investigation trainings.
    • KEY PERFORMANCE STANDARDS
    • Communication: Ability to communicate effectively, and demonstrate good interpersonal and professional skills when interacting with individuals served, co-workers, and management. Maintain open communication with your supervisor as it relates to the completion and accuracy of assignments. Listen and respond appropriately to others’ ideas, make suggestions and offer alterative solutions, and give and receive honest and constructive feedback.
    • Quality of Work: Assignments are accurate, complete, timely, and comply with agency and department policies and procedures. Independently work towards overcoming challenges and barriers to accomplish performance objectives. Complete an acceptable quantity of work within deadlines, while maintaining the required quality and accuracy of the work.
    • Integrity/Compliance: Uphold personal and professional ethics and values, considering the values of the organization and respecting the culture, beliefs and abilities of individuals we serve. Ensure compliance with HIPPA and other regulatory requirements for responsible areas, as well the completion of all assigned trainings.
    • Teamwork: Encourage team unity through sharing information/expertise, working together to solve problems, and putting team success first. Attend and participate in departmental and team meetings.
    • Self-Motivated/Flexible: Demonstrate initiative and endeavor to maintain a stable level of productivity. Ability to multi-task and balance competing priorities, work with minimal supervision, and exercise sound and practical judgment and problem-solving. Easily adjust to changes in routine, assignments, and agency/client needs while continuing to be productive.
    • Job Knowledge: Demonstrate skills and knowledge relevant to job functions. Apply current best practices and maintain awareness of major developments in your specialty area. Demonstrate expertise in relevant regulatory requirements.
    • Writing: Convey ideas and facts in writing using language the reader will best understand. Use correct vocabulary, spelling, grammar, and punctuation. Present information clearly, concisely, and logically for the intended audience.

    Qualifications

    • Bachelor’s Degree required in related field, together with minimum of two (2) years of experience as an investigator or a quality assurance staff member with a Human Services organization.
    • OPWDD and NYS Justice Center investigation certification, and/or the ability to obtain certification
    • Must be willing to:
    • Travel extensively in all of the boroughs
    • Work a flexible schedule in order to meet the needs of the department, including during non-office hours, evenings, weekends and holidays when necessary.

    PREFERRED QUALIFICATIONS & SKILLS

    • Master’s Degree preferred.
    • Investigation experience preferred.
    • In lieu of Bachelor’s Degree, High School Diploma or equivalent with minimum of seven (7) years of experience in the healthcare or non-profit field.
