Tri-Valley, Inc. is a private not for profit agency dedicated to enable older adults and individuals with disabilities to promote and maintain an optimal level of independence.

The Crisis Worker determines the need for protective intervention and the delivery or arrangement of services for self-neglected elders. The worker carries a case load at program standard.

Responsibilities:

• Accepts referrals from supervisor on appropriate cases requiring intensive and extensive casework.
• Conducts an assessment of each client referred and establishes an appropriate service plan.
• Maintains records regarding the case plan including physio-psycho-social characteristics and disposition of said clients.
• Utilizes community professionals who can act as consultants in developing case plans for clients in crisis.

Qualifications:

Must possess a Bachelors’ degree in Social Work Human Services or related field. Communicate with clients, co-workers, etc. both face to face and telephonically. Computer acuity. Ability to travel to appointments using employee’s own transportation. Driver’s license; motor vehicle record check and minimum insurance requirements. Maintain case load at program standard. Mileage allowance of 50 cents per mile for all Tri-Valley work related travel.
