Job Description
- Assist residents to identify, respond to, and cope with any stressors in their lives.
- Works to continuously assess resident’s needs and strengths in order to provide proper support and create attainable goals.
- Works a liaison between parents, day programs, job sites, physicians and therapists to ensure that everyone is receiving consistent information and the best possible care.
- Can also work as a mediator to help resolve any issues that may arise.
- Send out client satisfaction survey and document results.
- Schedule annual check-in meetings for individuals and guardians.
- Maintain communication among all residents and service notes.
- Host support meetings as needed
- Work with outside vendors to host group classes as needed.
- Work as a team with other social worker to maintain full understanding of the organization and be able to cover with each other
