We are a Recovery Friendly workplace, looking for a dedicated and compassionate Mental Health Case Manager to join our team. In this role, you will serve as an advocate for individuals on their mental health journey, helping them access government assistance, secure housing for the homeless, support client recovery, and more! Your efforts will make a significant impact on the lives of those facing severe mental illness.Compensation:
$19 hourlyResponsibilities:
Conduct comprehensive assessments to identify clients' needs and develop personalized care plansAssist clients in accessing and utilizing community resources, including mental health services, housing programs, and government benefits.Advocate for clients' rights and needs within various systems, including healthcare, social services and legal servicesProvide crisis intervention and support as neededCollaborate with other healthcare professionals, social workers, and service providers to coordinate comprehensive careMonitor clients' progress and adjust care plans as necessary to ensure optimal outcomesMaintain accurate and up-to-date client records and documentationEducate clients and their families about mental health issues, available resources, and coping strategiesParticipate in team meetings, training sessions, and professional development opportunitiesHold a valid driver's license and maintain transportation with automobile insurance coverage.Strictly adhere to HIPAA regulations and uphold client confidentiality.Meet productivity standards consistently.Maintain excellent attendance records.Qualifications:
