Day Habilitation Plan Coordinator

At Arc of Onondaga, Day Habilitation Plan Coordinators are responsible for overseeing the plan of services based on person centered planning concepts that are delivered to each assigned individual.
The Day Habilitation Plan Coordinator works with the person and their support team to develop and revise the habilitation goals with a focus on areas that are most important to the person, and is ultimately responsible for ensuring consistent implementation and documentation of all aspects of the individual's program plan.
