Summary

Care Team Coordinators (CTC) are responsible for organizing and directing care team tasks on a day-to-day basis to ensure the delivery of quality patient care. They are also responsible for all appropriate documentation and reporting requirements. Additionally, they partner with patients to meet their needs and ensure patient satisfaction.

Essential Functions and Qualifications

* Minimum two years working in a medical office setting.

* Punctuality and adherence to schedule are essential.

* Excellent problem-solving and time-management skills.

* Able to work collaboratively with multiple health professionals in a busy and complex environment using tact, diplomacy, and discipline.

* Strong charting/documentation skills.

* Excellent written and oral communication skills.

* Strong customer service skills.

* Ability to manage priorities and workflow

* Versatility, flexibility, and a willingness to work within a fast-paced setting with constantly changing priorities; all done with enthusiasm

* Acute attention to detail

* Strong interpersonal skills.

* Ability to work at a computer and on your feet for extended periods.

* Strong organizational skills and mindfulness

* Effective communication skills to interact professionally.

* Ability to manage confidential and sensitive information.

* Work is performed in a medical office setting using standard medical equipment. The risk of exposure to blood and bodily fluids is present.

Care Team Coordinator Responsibilities

* Acts as a single point of contact for the Care Team.

* Supervise care team inboxes to ensure they are clean and TEs are being addressed promptly by the entire team.

* Manages incoming care team phone calls.

* Take messages or transfer calls as necessary or send telephone encounters to the care team or provider/team member, as needed.

* Actively work Care Team telephone encounters; minimize the number of callbacks from other team members, whenever possible, within the scope of practice.

* Updates team throughout the day of outstanding tasks that need to be addressed.

* Acts as point of contact and liaison for OB patients.

* Manages patient requests for letters for school, work, and insurance paperwork.

* Meet with patient throughout pregnancy at designated intervals, and as needed.

* Point of contact to schedule a mammogram if needed for upcoming HRT appointment.

* Establishes a collaborative relationship with care team members and effectively communicates with patients and care team members.

* Backs up other care team coordinators, as necessary.
