Job Position: Bilingual Care Coordinator

Location: Chicago, IL 60608

Pay Rate: $20.00/hr - 22.00/hour

***Interested candidates must be Bilingual fluent in both English and Spanish***

Position Purpose: Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services .

Job Duties: 

• Develop, assess and adjust, as necessary, the care plan and promote desired outcome
• Assess member's current health status, review past and present treatment plan and services
• Coordinate services between Primary Care Physician (PCP), specialists, medical providers, etc.
• Develop plan of care based upon assessment with specific objectives, goals and interventions
• Facilitate member access to community based services
• Monitor referrals made to community based organizations, medical care and other services 
• Enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems

Education/Experience: 

• Bachelor’s Degree, Associates Degree or 3 years or more of health related experience.
• 2 years of clinical nursing experience in a clinical, acute care, or community setting. Knowledge of healthcare and managed care preferred. 
• Must be Bilingual fluent in both English and Spanish
• Knowledge of healthcare and managed care preferred. 
• Prior case management experience preferred. 

Certifications/Licenses:

• Current and valid CPR certification.
• Must be fully vaccinated and or willing to be vaccinated.
