The PACE Activity Coordinator is responsible for assessing the in-home and in-Center engagement needs of PACE participants in addition to collaborating on a plan for Day Center activities.
The Activity Coordinator is a member of the Interdisciplinary Team that is responsible for determining a coordinating plan of care for each participant and implementing that plan of care.
The Activity Coordinator is engaged in service delivery and service determination for the PACE program, and all-inclusive health plan for seniors.

The Program of All-Inclusive Care for the Elderly (PACE®) is a comprehensive health plan for those who require long term care services (like caregiving) and are living in the community.
PACE members are served by an interdisciplinary team focused on collaborating with members on a plan of care that helps them stay living in the community.
PACE programs are focused on delivering collaborative approaches to care and services that allow participants to live at home safely.

Position Summary
Responsible for providing leadership for the development and implementation of an effective, activity program for the participants at the PACE program which stimulate emotional, physical, and social skills in a group and home setting.
This position works under very limited supervision. Work situations are varied and frequently non-routine. This position requires independent thinking, creativity, conceptual planning and development skills, decision-making, use of good judgment and discretion, leadership, and strong interpersonal skills.
Internal contacts include departmental staff. External contacts will include participants, families / caregivers, physicians, and community agencies.
Requirements
Associate Degree in recreational therapy or a related field plus two years of appropriate experience; Bachelor’s degree preferred;
Combination of additional specific recent relevant experience, certification or program completion may be considered in lieu of education

As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position.
The pay scale is $24.33 - $35.01 USD. However, pay is influenced by factors specific to applicants, including but not limited to : skill set, level of experience, and certification(s) and / or education.
If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
