Hospice Social Worker (MSW)
Salus Hospice has an immediate opening and is presently seeking a seasoned hospice social worker to join our growing program!

General responsibilities include:
-Perform psychosocial assessment within specific time frame, including the assessment of social and emotional factors related to the patient's condition.
-Provide individual and family counseling; support through group counseling; provide assistance to staff and patient/family by investigating third party financial resources and planning for financial coverage for services.
-Provide care planning and evaluation/revision as appropriate to changing patient/family needs.
-Provide bereavement assessment & assess risk level.
-Professional experience working in geriatrics, hospice, or end of life care. Masters in Social Work required.
-Reliable transportation, license, insurance and willingness to travel regularly to patient locations.
-Other duties as assigned, for example assisting with the Volunteer and Bereavement Programs

Required experience:
-Candidates must have Masters in Social Work (MSW)
-One year experience in hospice required
