We are seeking to hire an experienced Case Manager bonus would be Certificated/Licensed professional who is looking to grow within a company.
Responsibilities:
ESSENTIAL DUTIES AND RESPONSIBILITIES
Serve as a member of IBH assigned multidisciplinary treatment team. Self-initiates and follows through on assignments in a timely manner. Participates in team meetings while supporting the discussed matters. Introduces new ideas/activities while problem solving for current dilemmas. Observes client behavior to identify changes or indicators of special needs and shares those observations with designated staff to carry out a necessary action plan.
● Facilitates group therapy sessions daily while then writing group notes the same day. 24 hours max to finish group notes
● Legal Communication:
o Immediately informs all legal contacts of client’s admission into IBH, including Probation Officers when ROI is signed;
o Provide biweekly/monthly progress updates to the legal contact, including urine analysis results if applicable (contact immediately after meeting with the clinical team if a positive test occurs);
o Provide clients with documentation for their Probation Officer/Judge in order to comply with legal responsibilities;
o Coordinate with Operations administrator for rides to court/probation appointments if applicable.
● Input documentation (PO forms, outside medical records, FMLA, disability) into Kipu, IBH electronic health records
● Coordination of Care
o Schedule doctor/health related visits for client’s for aftercare;
o Scheduling of various external appointments and when applicable, attending appointments with the client;
o Set up discharge planning (sober living, IOP, outside appointments,)
● Facilitates referrals and associations to community resources and services
● When needed, helps facilitate the Intake Process when a new client comes into the program.
This positional could also serve as a certified/licensed therapist
