Job Description:
As a Young Adult Care Coordinator at Ohana Family Homes, you will play a pivotal role in supporting young adults aged 18-21 in our Transitional Housing Program. You will work closely with our team, collaborating with the Arizona Department of Child Safety to ensure the holistic well-being and success of our young residents. Your responsibilities will include:
● Individualized Service Planning: Develop and write comprehensive service plans tailored to the unique needs of each young adult. These plans will encompass educational, physical, social, financial, and occupational goals, ensuring a holistic approach to their development and stability.
● Regular Check-Ins: Conduct frequent check-ins with young adults to assess their progress, address challenges, and provide necessary support. These check-ins will serve as opportunities to build rapport, trust, and a supportive relationship with each resident.
● Life Skill Development: Facilitate life skills activities to equip young adults with essential skills for independent living. Topics may include budgeting, job readiness, communication, conflict resolution, and self-care.
● Creating Normalcy: Foster a supportive and nurturing environment where young adults feel valued, respected, and empowered to lead fulfilling lives. Help cultivate a sense of normalcy and stability amidst the transitional nature of their housing situation.
● Collaboration: Work collaboratively with the Arizona Department of Child Safety and other relevant agencies to ensure coordinated and effective support for young adults in our program. Foster strong partnerships and communication channels to maximize resources and opportunities for our residents.
Qualifications:
● Bachelor's Degree preferred, in a related field such as Social Work, Psychology, Counseling, or Human Services.
● Experience working with teenagers or young adults, particularly those who have experienced trauma or adversity.
● Knowledge and understanding of trauma-informed care principles and practices.
● Strong communication and interpersonal skills, with the ability to build rapport and establish trust with diverse populations.
● Organizational skills and attention to detail, with the ability to effectively manage multiple tasks and priorities.
