The Primary Care Provider is responsible for diagnosing and treating patients in a kind and compassionate, care-centered, outpatient environment:
The Nurse Practitioner or Physician Assistant job requirements consist of:
Assessment, diagnosis, prescription, selection and administration of therapeutic measures and treatment regimes
Consult, plan and implement collaborative management, referrals, or transfer of care as appropriate.
Practice within standards consistent with those established by the New Hampshire State Board for his/her license.
Recognize limits of knowledge and experience and plan for the management of situations beyond own expertise
Maintain up to date and timely records and documentation.

Allocate sufficient time as is reasonably necessary for effective patient care, including direct patient care and administrative/documentation work; typically, 40 hours per week.

Maintain confidentiality as it applies to all state, federal and company guidelines.
The Nurse Practitioner or Physician Assistant will provide a variety of services including:
Consultation and referral
Routine office visits
Medication management
Referral to specialists when appropriate
Collaboration and support across provider group
On Call Rotation
Experience preferred:
Candidate must be licensed or able to obtain a license in the State of New Hampshire.
Candidates seeking part and/ or full-time positions will be considered.
Pediatric experience is a plus as we are seeking additional support on our Peds team.
