Schedule: Monday through Thursday 11 a.m. to 7:00 p.m. Friday 9:00 a.m. to 5:00 p.m.

About the Role:

The LPN will perform telephonic patient outreach and is responsible for interviewing patients about their medications related to prescriptions and over-the-counter items. Topics of discussion include adherence, adverse effects, current health status, and therapeutic recommendations. No Hands-on Care.

Key Responsibilities:

  • Must be bilingual speaking both Spanish and English fluently.

  • Maintains current knowledge of medications.

  • Perform telephonic medication reviews.

  • Accurately document clinical data in patient charts.

  • Identify potential issues based upon information gathered during the interview with the patient.

  • Follow all established CSS Health policies and procedures including but not limited to HIPAA, privacy and security policies and procedures.

  • Operate office equipment such as voice mail messaging systems, email, and use of various software applications to support operational processes.

  • Maintain appropriate training levels for duties and responsibilities described.

  • Perform other duties or tasks, as assigned, or required.

  • Must have high-speed internet.

Qualifications:

  • Graduation from an accredited nursing program (LPN) with an active license.

  • At least 1 year of experience in the healthcare industry is preferred.

  • Prior call center experience preferred.

  • Ability to work as a team member, be professional and have telephonic communication skills, have attention to detail, and exercise professional work ethics.

  • Understanding of Medicare and Medicaid programs preferred.

  • Understanding of HIPAA, Privacy, Safety, and Compliance guidelines.

  • Knowledge of MTM and Transition of Care procedures and processes preferred.

  • Intermediate understanding and ability to navigate a database system.

  • Intermediate understanding of Microsoft Office Suite (Excel, Word, etc.) and knowledge of efficient record-keeping techniques.
